Job DetailsJob Location: Omaha, NE 68107Education Level: Associate's Degree (Two-Year Degree)One full-time position | Monday - Friday 8:00 am - 5:00 pm | Livestock Exchange Campus (30th & L) Job Duties: The diabetes care and education specialist (DCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes care. The DCES is an integral part of the interprofessional team and provides collaborative, comprehensive, and person-centered care and education conducive to behavior change and improved quality of life across the lifespan. The DCES delivers case management services to patients to ensure cost-effective use of resources and positive patient outcomes. The DCES supports and advocates for people affected by diabetes to optimize quality care. Integrates skills and knowledge of pathophysiology, clinical management, motivational interviewing, and self-management of diabetes and the American Diabetes Association’s Standards of Medical Care in Diabetes to clinical practice.
