Job description

Position Summary: Eventus WholeHealth delivers an integrated model of care to adults who reside in a variety of settings including but not limited to skilled nursing facilities, assisted living facilities, independent living and in personal homes. These services are provided through a network of Eventus WholeHealth providers who include Physicians, Nurse Practitioners, Physician Assistants, Care Team Advocates, and in-house Support Staff. Telemedicine uses videoconferencing equipment to send and receive patient medical information and pictures. Eventus WholeHealth telemedicine providers deliver medical services to patients at assigned contract sites according to site-specific protocols. Telemedicine providers also render medical consultations with family, facility staff and specialty providers when necessary. Medical services are provided via telemedicine.
