What we have to offer: Position Overview: This position works in the Pediatric ICU, providing discharge coordination amid relentlessly high demand. This position is critical to throughput in medical floors. Tasks include family meetings, care coordination, referrals, securing community appointments, and collaborating with community agencies, programs, groups, etc. The SW is the liaison between the team and the family for a fast-paced, high-stakes, sensitive, and intense set of scenarios and needs.
