Key Responsibilities: Client Care and Treatment Support: - Serve as the primary point of contact for clients regarding medical treatment, care coordination, and ongoing support throughout the treatment process - Assist clients in navigating treatment recommendations, referrals, scheduling, and continuity of care - Develop and manage individualized care coordination plans based on client treatment needs and case progression - Identify and facilitate referrals to appropriate specialists and providers as needed - Coordinate communication between clients, medical providers, and legal teams regarding treatment status and ongoing care needs - Proactively identify and help resolve barriers, delays, missed appointments, or breakdowns in care that may impact treatment progression or case development - Support continuity of care, client engagement, and overall treatment progression throughout the lifecycle of the case Documentation & Record Management: - Maintain accurate, organized,...
