Overview Based within the Central Health Navigation Center, the LVN Navigator Lead oversees and supports the Central Health Transitions of Care programs and additional services. This role supports a team of Medical Assistants (MAs) and Licensed Vocational Nurses (LVNs), serving as the primary contact for patients, healthcare providers, and community partners. The LVN Navigator Team Lead ensures a seamless transition for patients discharging from hospitals or other acute care facilities to post-acute care settings. Responsibilities include managing referrals, supporting discharge planning, and providing telephonic assessments, coaching and support to facilitate comprehensive healthcare system navigation. The Lead ensures proper referral transcription and navigation to the appropriate service lines. Additionally, this role involves assisting patients through inbound and outbound calls, guiding them through the complexities of the healthcare system.
