Job description

The Pediatric Hematology/Oncology Clinical Pharmacist will provide pharmacotherapeutic support to the pediatric hematology/oncology service of Goryeb Children’s Hospital and Valerie Fund Children’s Center in both inpatient and outpatient settings. In addition to serving as a member of the rounding interdisciplinary care team, the Pediatric Hematology/Oncology Clinical Pharmacist will take an active role in the education of patients, caregivers, and members of the pharmacy and interdisciplinary team(s). This pharmacist will participate in multidisciplinary committees and will also serve as a preceptor in the Atlantic Health PGY1 Pharmacy Residency Program. Additional responsibilities include: - Provide comprehensive pharmaceutical care to pediatric oncology patients, including: review and recommend optimal, evidence-based medication therapy; provide pharmacokinetic consultations as required; document any changes in patient and pharmacy records and inform others of medication changes; prevent, detect, monitor, document, and report adverse drug reactions and medications errors; participate in the drug therapy management of medical emergencies; promote the use of the formulary by conve...
