Including, but not limited to, daily assessment of their physical and mental state and upkeep and accuracy of their charts. BRIEF OUTLINE OF POSITION: 1. Responsible for taking vital signs, assessing the client’s medical and psychological symptoms, and documenting the same in the daily nursing progress notes. 2. Ensure proper and accurate transcription of physician orders and accurate documentation on the medication administration record, verbal physician orders, and individuals on self-medication plans. 3.
