Job DetailsJob Location: Yakima, WA 98901Position Type: Full TimeEducation Level: Associate DegreeSalary Range: $22.00 - $27.00 HourlyTravel Percentage: Up to 50% Job Shift: DayPosition Summary: The Case Manager provides case management, care coordination, support and other various clinical services for clients receiving outpatient behavioral health or social services. This role assesses client needs, develops individualized service plans, and connects clients with appropriate community resources under the direction of the Clinical Supervisor, as needed. Responsibilities: Direct Treatment Services Provides case management and therapeutic services, including education and referrals to individuals who meet access to care criteria Collaborates with clients to develop and carry out individualized treatment plans, focusing on client strengths, goals and addressing any risk factors Completes progress notes and other required documentation within the required timeframes.
