Description JOB SUMMARY: Within the context of a client and family centered care model and in accordance with established standards of practice, and vision and values of the organization, works as a member of an interdisciplinary team to conduct the intake functions for the designated treatment programs to ensure linkages between client and the appropriate resources occurs. Performs initial screening and assessment, triage and referrals based on client needs, urgency of request and best practices to determine the appropriate treatment and educates and provides consultation to client/family. Provides information to health providers and the general public, establishes and enhances relationships/communication within the programs and other community resources, prepares and maintains client records, maintains and provides accurate intake/referral data and statistics, participates in team meetings and continuing education, and engages in on-going team planning and quality improvement. QUALIFICATIONS: Education & Experience Baccalaureate Degree in a related Health Science discipline (such as Counseling, Social Work).
