Role: Case Manager Reports to: Assistant Director LEAD Program: 1602-DHS ODR Department: Permanent Housing Location: Campion 1447 16th Street, Santa Monica, CA 90404 Setting: 100% Onsite Schedule: Monday - Friday, 8:00am - 4:30pm Status: Full-time/Non-Exempt/Non-Management Benefits: Medical, Vision, Dental, Life Insurance, 403(b) Retirement plan, Employee Assistance Program (EAP), etc. Openings: 1 Job Description Summary: With the support of and in partnership with the LEAD team, Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals, and is responsible for identifying, engaging and providing support to homeless clients referred from the LAPD Pacific division, including aftercare services and follow up support, once housed. The goal of LEAD is to connect with clients experiencing frequent contacts with police due to their homeless status and enroll them in services to prevent entry to the jail and criminal justice system.
