Reliance Hospice and Palliative Care is looking for a compassionate RN Case Manager to join a strong supportive team of professionals that provide hospice/palliative care around South Orange County with cities not limited to Laguna Niguel, Lake Forest, Rancho Santa Margarita, down to San Clemente. Hourly Salary starts at $52.00 and is negotiable depending on experience. Sign-On bonus only available for those who can start immediately. Job Summary: The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
