Case Manager (Unaccompanied Minors Program)

Job description

The Case Manager works under the direction of the Program Director and is responsible for aiding in the delivery of case management and clinical treatment services and assistance in coordinating program operation. RESPONSIBILITIES: QUALIFICATIONS: HOURS OF WORK: M-F 8:30-5 nights and weekend and on call availability BENEFITS: - Ensure that all youth receive services while in care including but not limited to mental health, medical, recreation, education, and other support services - Maintain accurate, and current information on foster children in care, while ensuring ORR reporting requirements are met. - Coordinate ongoing searches and connection with family or significant adult sponsor (Instead of HV language) - Assist the Clinician/Consultant in providing other case management services, including scheduling client’s activities, and making appropriate contacts with referring workers, program staff, etc. - Assist the Clinician/Consultant in Case Reviews by sending out invitations, compiling test results, attending, and participating in Case Review Meeting discussions, and writing summaries. - Assist the Clinician/Consultant in processing case related information, including profile...
