Assistant Director - Population Health

Job description

Responsible for directing strategy and operations for population health programs, to improve the health of SBH patients and our community. Provides strategic direction, leadership and vision to program teams, overseeing daily operations and management. Supports the Vice President to improve care quality, strengthen relationships and grow business. - Directs strategy and daily management of assigned program operations and workflows. - Directly supervises and manages assigned program staff on a day-to-day basis, interviews and hires, evaluates and documents staff performance and appraisals. - Manages all administrative functions, coordinating daily operations, and communicating changes to workflows, and policies and procedures - Leads continuous improvement of assigned program processes and patient experience.
