Position Overview Provide Peer support services to program participants, conduct educational and support groups, and plan and implement outreach events. Primary Department, Division, or Unit: Primary Work Shift: Day Regular Scheduled Weekly Hours: 40 Compensation Range: $17.05 - $22.17 per hour, depending on experience Provide support and collaborate with the LGBTQI+ team by assisting with the coordination of program activities and day to day operations. Collaborate with Primary Care providers and the team in assessing, planning, coordinating, and managing the biopsychosocial needs of individuals for care services and patients. Support the case manager in delivering support services to patients to improve patient care for the LGBTQI+ population. Work with the case manager to coordinate and provide case management services to program participants and assist with appointments, client outreach calls, and utilize lived experience to assist patients as they move through their care and treatment process.
