We are seeking a highly motivated and empathetic individual to join our team as a Mental Health Care Coordinator. The primary responsibility of this role is to launch a new clinical program that supports high-risk patients. The successful candidate will work closely with healthcare providers to coordinate and facilitate the implementation of individualized care plans for patients with high symptom severity. This is a fully remote role, overseeing the Care Coordination for our offices in Texas.
