Job Description: - Based at St Georges Hospital Kew - Friendly and supportive team environment - Full time fixed term until 24.12/26 About the Role: You will provide a comprehensive Occupational Therapy service to the Transition Care Program clients both bed based and in the community. Duties: · Provide high quality and optimal occupational therapy assessment and clinical management of Transition Care Program clients. · Plan, implement and monitor appropriate treatment programs from established client goals using best practice principles and evidence-based practice. · Provide carer/family support to assist in the management of Transition Care Program clients at home.
