PCHC’s Care Management Team is seeking a Community Health Worker to join our Housing Outreach & Member Engagement (HOME) team. In this role, you’ll deliver peer recovery and support services to adults and youth who meet the MaineCare definition of chronic homelessness. You’ll serve as a trusted role model and advocate, helping clients build independence and take ownership of their recovery journey—acting as a vital bridge from the street to stability by connecting individuals with essential resources, healthcare providers, and community supports while cultivating meaningful, trusting relationships along the way.
