Case Manager – Enhanced Young Adults Program The Case Manager is responsible for coordinating case management services, linking clients to community-based resources, and providing the necessary education and support to help young adults maintain stable housing. The role involves developing individualized support plans and fostering connections to essential services. Duties include 1. Assess clients’ needs and strengths for services in the community and eligibility for all funding sources. Connect clients to all appropriate funding sources upon admission to program. Coordinate completion of entitlement forms and submit within time deadline. 2. Identify any immediate crisis intervention needs and services. Develop a support plan. 3. Provide referral and linkage to community services as needed. 4. Assess, refer, and provide information on basic needs. Assess the transportation needs of the client and provide referral and education regarding available transportation. 5. With clients, develop a schedule of visits and provide continual adjustment to the number of visits based on the clients’ needs. 6.
