UNIT PROFILE: The Home Dialysis Program provides comprehensive and collaborative education, assessment, and ongoing support for patients and families/support persons who perform hemodialysis or peritoneal dialysis in the home setting. Nurses in this program promote patient independence through consistent, evidence informed teaching and clinical support whether during clinic follow up appointments, at patient’s homes or when providing dialysis training sessions. Care is provided in alignment with unit policies, provincial regulatory standards, and best practice guidelines for home based renal replacement therapy. POSITION SUMMARY: The Registered Nurse (RN) is a regulated health care professional who practices in accordance with the Standards and Guidelines of the College of Nurses of Ontario, as well as the mission, vision, and values of St. Joseph’s Healthcare Hamilton.
