Job DetailsJob Location: Ganado, AZ 86505Position Type: Full TimePosition Summary: The purpose of this position is to manage the Special Diabetes Program for Indians (SDPI) Grant and coordinate program activities. This also includes providing patient care specifically to the Sage Memorial Hospital diabetic population and promoting comprehensive diabetes care for patients. This position is responsible for promoting SDPI Best Practices and a systematic approach to diabetes care. Under general supervision of the Chief Community Health Officer, Community Health Clinical Leadership and in collaboration with the OPD Medical Director, accomplishes the Special Diabetes Program for Indians (SDPI) program’s strategic objectives by planning, organizing, and implementing the assigned functions required to operate and maintain departmental activities and services. Ensures quality, patient care delivery and maintains confidentiality of all privileged information.
