Job description
The Hospice RN Weekender reports to the Team Manager. He/she is responsible for identifying patient/family care needs, and for providing nursing and supportive care for the terminally ill patient and their families. Education: Diploma, ADN and/or BSN from an accredited institution Licensures/Certifications: Experience: One to two years of nursing. Home Health or Hospice experience preferred. Skills: - Current Maryland State license as a Registered Nurse - Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection - Strong time management skills - Excellent interpersonal and communication skills - Ability to be patient and family focused - Skill in the use of EMR (Electronic Medical Record) and/or ability to learn - Skill in the use of Blackberry and portable computer Principal Duties and Responsibilities: - Completes initial assessment of patient and family to ascertain home care needs - including physical, emotional, spiritual and mental status. - Prepares and implements the care plan for patient/family unit in cooperation with the Interdisciplinary Group. - Provides direct nursing care to patients in order to maintain hig...
