POSITION SUMMARY: The Case Manager will support the needs of patients in a new, 45-bed men’s behavioral health unit operated by Boston Medical Center (BMC) located in the Suffolk County Sheriff’s Department’s (SCSD) Jail. Serving as a liaison with patients to facilitate access to care and gathering necessary information to inform treatment decisions, the case manager will be a critical partner for clinical and administrative staff to provide intensive behavioral health services to incarcerated persons. They will assist patients in addressing unmet social factors as health such as housing, employment, state benefits and support with reentry needs including referrals to community based treatment. This role requires experience in navigating the substance use continuum of care. The role requires assertive outreach, advocacy, system navigation, and service coordination to manage barriers that may impede patients’ access to clinical services and progress in their recovery. Excellent interpersonal skills, patient engagement skills and the ability to work independently and collaboratively are key requirements of the job.
