Job description

The Certified Nursing Assistant will be under the direction of the Registered Nurse or Therapist, provide personal care services. This position is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Participates and is an active member of the patient’s interdisciplinary treatment team. Job Responsibilities: - Provide care as defined by the RN/PT/OT/ST and a Home Health Aide Care Plan. - Communicates status/condition changes to the RN/PT/OT/ST per agency guidelines. - Promotes personal safety and a safe environment for clients and coworkers - Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist such as range of motion or assistance with ambulation or exercise. - Assists/provides patient with ADLs to include bathing, shampooing, skin/nail care, oral hygiene, shaving and/or dressing. - Performs household services essential to health care at home, including but not limited to Meal preparation, laundry or light housekeeping. - Demonstrates safe practice in the use of equipment.
