Recconect Case Manager

Job description

About the role The Reconnect Practitioner supports young people aged 12–18 who are homeless or at risk of homelessness by providing early intervention, case management, and family-focused support. The role works closely with young people and their families to strengthen relationships, address challenges such as disengagement, mental health, and conflict, and connect them with education, employment, and community services. Working collaboratively with internal teams and external partners, the practitioner advocates for young people’s goals while ensuring coordinated, safe, and tailored support to help them achieve long-term stability and positive outcomes. This is a Permanent part-time opportunity based in Geelong within our Family, Community and Disability Program.
