Position: Clinical Lead –Murray PHN Chronic Disease Exercise and Education Program Location: Across NCN Health: EFT/Hours: 0.6 EFT (48 Hours Per Fortnight) Status: Part Time Fixed Term until June 30, 2027 Hourly Rate: Dependent upon qualification and skill level and 12% Superannuation About the Role: NCN Health has an exciting opportunity for a Clinical Lead (Allied Health or Nursing) to join our Allied Health and Chronic Disease team. The Clinical Lead will work under the direction of the Clinical Coordinator and is responsible for the implementation and facilitation of the newly funded Murray PHN Chronic Disease Exercise and Education Program. This program is funded for 12 months and is in partnership with Yarrawonga Health. The program will deliver direct clinical services through a structured exercise and education group program for people living with chronic conditions - cardiovascular (CVD), chronic obstructive pulmonary disease (COPD), and diabetes who reside within the Moira Shire.
