Franciscan Health Crawfordsville Campus 1710 Lafayette Rd Crawfordsville, Indiana 47933 The Home Health Occupational Therapist (OT) evaluates and implements a plan of care to improve, restore and enhance physical functional performance and promote injury prevention. This role also performs documentation and quality improvement activities as outlined in departmental policies and procedures. Document patient assessments and pertinent information in a timely manner. Develop a treatment plan with measurable goals based on patient assessment.
