Description JOB TITLE: CHR Transporter and Assistant (Rossville, KS Part-time) ORGANIZATIONAL UNIT: Health Services: Department: Senior Support Network: SUPERVISOR: SSN/CHR Coordinator: FLSA: Non-exempt: SALARY: BOE $9.00-16.00: **THIS POSITION IS LOCATED IN ROSSVILLE, KANSAS**: JOB SUMMARY: Transporting Tribal Elders to medical appointments or other destinations as needed by the Senior Support Network. Offering assistance to the Elders as needed to get to their appointments. Follow the CPN CHR Transport policy. Assist the CHR nurse as needed. CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening. ESSENTIAL FUNCTIONS: · Provide Tribal Elders safe transportation to and from medical appointments or other destinations as required. · Give assistance as needed to get the Elder to their appointments--this may include helping them with their assistive devices or with any paperwork they are given at their appointment. · Document all mileage, addresses, and client signatures as required on Transport Logs. · Promptly report any problems encountered with th...
