Job Description The Nurse Practitioner within the Nurse-Led Outreach Team (NLOT) program utilizes advanced practice nursing expertise to provide comprehensive clinical care. This includes advanced assessment, diagnosis, and treatment, as well as education, consultation, and research in the field of Geriatrics. The position plays a vital role in improving access to timely, evidence-based medical care for residents in long-term care homes (LTCHs) within the Mackenzie Health Hospital catchment, decreasing avoidable ED transfers and hospital admissions, and advancing a more integrated, resident-centered model of care delivery. This role will positively impact organizations, improving system partnerships, increasing capacity and efficiency through decreasing wait times and length-of-stay, and facilitating enhanced information exchange.
