Job description

Description Under the supervision of the Regional Services Manager, performs assigned duties relating to the implementation and evaluation of community health worker services. Client Services: · Engage in outreach activities at various community sites to identify clients in need of case management services. · Assess client needs, prepare appropriate individual service plans and maintain records. Assist clients in implementing the service plan by networking and referral, internally and/or to other community resources. · Conduct referral and linkage activities to housing, food programs and other customer needs. · Develop and maintain database of professional contacts. · Establish, maintain and update client case records using approved agency forms and procedures. ensure confidentiality of all records. · Provide relevant literature to clients and members of the community. · Coordinate with social services and insurance companies or other financial services to ensure ability to pay for care. · Assist with insurance and benefit applications · Advocate for clients who have legal benefit concerns Assist homeless or precariously-housed clients towards more stable housing Reporting / Monito...
