Job Type Full-time Description: Position Overview: The social worker will work with residents in the skilled nursing facility by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. The Social Services Director shall provide residents with the highest practical level of physical, mental and psychosocial well-being and quality of life. Key Responsibilities: - As a social services director, you will oversee the planning and implementation of social services programs in your facility. - Recruit, hire, train and supervise staff, including MHPs, QMHPs, CRSs, and RSAs - Experience with evidence based treatment modalities including wellness recovery action plans, illness management and recovery, dual diagnosis, and trauma informed care - Oversight of the implementation of treatment programs - Coordinate referrals of residents to services the facility does not provide - Develop and coordinate educational in-services - Provide clinical case supervision of all treatment personnel - Oversee the assessment of all potential residents - Develop Performance Improvement Project (PIP) tools to impro...
