We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surronding areas. This position offers a rewarding opportunity to make a significant impact in the lives of those we serve. Key Responsibilities: - Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans. - Provide emotional support and education to patients and families about end-of-life care and self-care techniques. - Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress. - Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care. - Maintain accurate clinical documentation and participate in performance improvement initiatives.
