Position Overview: The Value-Based Care Manager is responsible for coordinating and managing the care of oncology patients participating in value-based care programs. This role serves as a central point of contact for patients and caregivers, ensuring care coordination, reducing duplication of services, and supporting positive clinical and cost-effective outcomes. Working collaboratively with physicians, nurse navigators, and interdisciplinary teams, the Value-Based Care Manager helps patients navigate the healthcare system while ensuring compliance with program requirements and practice redesign initiatives.
