Responsible for coordinating a patient-centered, interdisciplinary plan of care for patients in the acute inpatient care setting. Work collaboratively with internal and external care providers, facilities and agency partners to ensure a coordinated post-acute discharge or treatment plan is in place so to support a seamless transition of care.
