We are seeking a full time Case Manager to join our Dual Diagnosis Treatment Team in our Somerset, PA location. Earn $24.38/hour. Position Details: The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
