Whittier Rehabilitation Hospital - Transitional Care Unit/SNF - Haverhill MA: Unit Manager - RN 20 Bed Transitional housed within our Rehabilitation Hospital Regular Schedule will be M-F days (40 hours), with coverage as needed on weekends and off-shifts. We are seeking a Nurse Manager to oversee and lead our Transitional Care Unit housed within our beautiful Rehabilitation Hospital on Ward Hill in Haverhill Ma. Essential Duties : To provide exceptional care to all residents including but not limited to: - -Under the direction of the Director of Nursing Services, holds 24-hour authority, responsibility and accountability for the clinical management of assigned resident/patient care unit. - - Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary - - Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary - - Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action - - Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations d...
