Are you a CNA with a valid Louisiana driver’s license and transportation? If so, PACE Lafayette Rouge invites you to join our compassionate and dedicated team as an Adult Care Assistant. $1,500 sign on bonus for qualified candidates!!!! Responsible for assisting the needs of the Home Care Coordinator which include communication with Personal Care Contracted Companies and DME Contracted Companies. Assists in Setting up Personal Care Services in the home and ordering and tracking DME and supplies. Assists in the coordination of care between the families, personal care companies and transportation. Performs telephone audits with the participants receiving personal care in the homes. Experience - One year working with the frail elderly. - Communication - Communicates with the Home Care Coordinators on a routine basis to assist in coordination of the care of the Participants. - Communicates to the Personal Care Company the needs of the Participant for Personal Care.
