About Us Seeking a bi-lingual Consortium and Outreach Coordinator to join the Bronx Healthy Start Partnership team. The Bronx Healthy Start Program aims to address gaps in birth outcomes in Bronx County, through the provision of direct support to families during pregnancy and postpartum and engaging partners in the broad mission of achieving birth equity with a special focus on the social determinants of health. The goal of the position is to facilitate and support a coalition of partners to address the social determinants of health and achieve systems changes that improve birth outcomes. Under the direction of the Project Director, the Consortium and Outreach Coordinator operates cooperatively as part of a multi-disciplinary team to convene and facilitate a robust Community Consortia (formerly known as Community Action Networks or "CANs") comprised of diverse multi-sector partners that advise and inform BxHSP activities, as well as develop and implement action plans to improve perinatal outcomes within Bronx County.
