This Position is eligible for $2,500 retention bonus. Job Summary: The Providence Center Residential Case Manager Ledge Street Apartments will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Work with consumers to achieve the highest level of independence possible. Duties and Responsibilities: Initiate treatment and advocate for consumer within network of care. Negotiate and schedule appointments for consumers within TPC and with outside agencies (Social Security Supplemental Insurance (S.S.I.), Food Stamps, Advocates Office, Medical Doctor, etc.) Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, consumers themselves. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
