Essential Functions Statement(s): ● Manage patient care and scheduling. ● Maintain accurate, detailed reports and records. ● Process, validate, and analysis of OASIS. ● Create 485 from OASIS assessment. ● Modify patient treatment plans as indicated by physician orders and patients' responses and conditions. ● Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans. ● Monitor all aspects of patient care, including POC being carried out as written and goals met. ● Instruct individuals, families and other groups on topics such as health education, disease prevention, and develop health improvement programs. ● Ensure the team is educated on the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems. ● Inform physician of patient's condition as required. ● Observe nur...
