Description JOB SUMMARY: Within the context of a client and family centered care model and the vision and values of the organization, the position develops, implements, coordinates and evaluates the goals, objectives and policies of the designated seniors and/or social service program(s). Supports and enhances the health, independence and quality of life of seniors in existing seniors housing, improves linkages to community, health, and social services, develops and delivers opportunities for social interaction and recreational activities and fosters partnerships with other community and health resources. Works collaboratively with elderly tenants, building managers and health and social service agencies to plan and implement activities and services for the benefit of tenants.
