We are seeking an experienced Registered Nurse (RN) to join our team as a Minimum Data Set (MDS) Coordinator in our Health Center. Schedule: - Coordinate and complete MDS assessments and care plans - Ensure accuracy and timeliness of documentation and compliance - Lead and participate in care plan meetings with residents and families - Collaborate with nursing, therapy, and interdisciplinary teams - Monitor quality indicators and support performance improvement initiatives - Partner with billing/finance to support Medicare and reimbursement accuracy - Support regulatory compliance and survey readiness - Full-Time position - 8:00 AM-4:30 PM (Primarily day shift hours) - On call rotation as needed (weekends as assigned) Why This Role Stands Out: - Primarily day schedule with consistent hours - Opportunity to step into a leadership-focused, non-floor role - Collaborative interdisciplinary team e...
