Senior Population Health Coordinator

Job description

Overview This position is onsite in Lebanon, NH. Monday - Friday, 8:00AM - 5:00 PM. With minimal support from supervisors, coordinates multi-stakeholder internal and/or external partnerships and project teams to plan and implement population health initiatives focused on Substance Misuse Prevention and Mental Health supports for youth. This role will work with schools and other community partners to support suicide and substance misuse prevention strategies for youth and young adults in the Greater Sullivan County area. The role is four days a week in person in the Sullivan County area, and one day a week is remote work. Responsibilities: - Leads development of project/ program related assessments activities with minimal supervisor support. - Facilitates, develops, coaches, and provides technical assistance and best practices for one or more collaborative partnerships. - Facilitates or leads project planning efforts with internal and external stakeholders. - Leads, supports and coordinates efforts of internal and external stakeholders to implement evidence based practices, activities and programs. - Coaches and mentors departmental peers and junior positions as well as internal/ext...
