Description: Work Schedule: 90% FTE, day/evening, 10-hour shifts, every other weekend, and holiday rotation required. Hours are based off the operational needs of the department. This position is hybrid with onsite work, remote work and travel required within the Madison, WI area. Your assigned work location will be the Administrative Services Building at 301 S. Westfield Rd., Madison, WI. Be part of something remarkable: Come coordinate care for Home-Based Hospital Care (HBHC) patients, serving as the central point of contact through virtual visits and phone communication to ensure seamless, hospital-level care in the home.
