This role supports youth both in placement and in the community by completing comprehensive assessments, developing individualized reentry plans, facilitating skill-building, and coordinating access to critical services such as housing, employment, education, and behavioral health care. The Case Manager works collaboratively with youth, families, Hill Country treatment teams, and justice-involved partners to promote successful transition planning and long-term stability.
