Job description

Provide physical therapy services to clients in their home, under the direction of a physical therapist. Work Schedule: 1.0 FTE; M-F 8-4:30pm This position will be covering our Wisconsin side, which is the western part of St. Croix County- northern part of River Falls, Hudson, Hammond, Baldwin, New Richmond, Somerset, Deer Park, Houlton, etc.; paid time to drive and paid mileage. Required Qualifications: Ability to read, write, and speak the English language - Graduate of accredited physical therapy assistant program - Continuing education as required by state licensing boards - Must have MN or WI Physical Therapy Assistant (PTA) license upon start date. - Must provide proof of MN and WI Physical Therapy Assistant (PTA) license within 3 months of start, if both MN and WI Physical Therapy Assistant (PTA) licenses are not present on start date - Must have a vehicle with auto insurance and a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines - Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider) Preferred Qualifications: Proof of application for alternate state...
