Job Title : Care Navigator - BHN Department : G1 Nursing: Location : Rochester Regional Hospital - 1425 Portland Avenue, Rochester, NY 14621 Hours Per Week: 36: Schedule : Day/Evening/Night, 12 hour shifts, Start times include: 7a, 11a, 3p, 7p, 11p SUMMARY: The Care Navigator functions as a core member of an Integrated Care team that includes the Primary Care Provider (PCP), Behavioral Health Clinician (BHC), and psychiatric provider. The Care Navigator is responsible for coordinating and supporting Mental Health care within the clinic and for coordinating referrals to clinically indicated services within and outside of the clinic. The Care Navigator may provide evidence-based treatments to patients or collaborate with other Mental Health providers when such treatment is indicated. This role will report to the Manager, Behavioral Health/Primary Care Integration or designee. RESPONSIBILITIES: - Support and coordinate behavioral health care with the patient, patient’s Primary Care Provider (PCP), BHC, psychiatric provider and other Integrated Care team members. - Screen and assess patients for common mental health and substance abuse disorders.
