Job description

Job Description The ED Care Coordinator is responsible for effective communication with physicians, nurses, and all ancillary departments to ensure continuity of patient care and maintain efficient flow of the department. The ED Care Coordinator is also responsible for effectively communicating with outside facilities (hospitals, transportation agencies, physicians, etc.) to coordinate transfer of patients from the ED for tertiary care or higher level of care facilities. SHIFT AND SCHEDULE: Full Time, Days, 11:00am-11:00pm: ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS: - Answer multiple phone lines and transfer calls appropriately while exhibiting professional phone etiquette. - Answer patient call lights as appropriate and communicate patient needs with appropriate staff members in a timely manner. - Assist with plan of care coordination to include obtaining patient information, completing documents, and documentation in electronic health record to ensure timely throughout. - Demonstrates proficiency in the ability to navigate the electronic healthcare record (EHR) and other various electronic programs to obtain patient information and provide timely, accurate documentation. - Pro...
