Job Summary: The Coordinator of the TAVR (Transcatheter Aortic Valve Replacement) Program utilizes discretion and independent judgment to facilitate seamless care from referral to follow-up for patients who are candidates for TAVR therapy; applies nursing process to the provision of care for this subset of patients; manages and coordinates all aspects of the TAVR program; facilitates a team approach and collaborates with the multidisciplinary TAVR team to establish a plan of care starting from initial interventional evaluation and following the continuum of care including pre-procedure, post-procedure, and continued follow-up post TAVR implant; works with the patient and family to provide teaching, education and support throughout the TAVR process. Serves as a liaison among physicians, staff, patients, families, and payers to ensure effective utilization of resources and coordination of care. Ensures maintenance of established electronic database, and submits patient demographic, medical and progress report information within designated time frames as required by local, state, and federal agencies.
