Employment Type: Part time Shift: Day Shift Description: Per Diem Social Worker - Hospice Columbia and Greene Counties The Community Hospice has a Per Diem opportunity for a social worker to join our hospice care team caring for patients in primarily the Columbia County area as part of our Interdisciplinary Team in homes and Nursing homes. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff.
