Job DetailsJob Location: 415 Columbia Rd - Dorchester, MA 02125Position Type: Full Time 30+ hoursEducation Level: 4 Year DegreeSalary Range: $19.00 - $26.00Travel Percentage: NegligibleJob Category: Health CarePay Range: The expected base pay for the position is $19-$26 per hour and may be increased based on other factors, such as language, certifications, etc. Under the supervision of the Director of Nursing, the Community Health Advocate provides support and linkage to services, advocacy, education, and information in order to ensure optimal health outcomes, increase access of services, and support the patient in self-management of health goals. The Advocate also serves as a cultural bridge to the health care system aiding in identifying and supporting the patient’s cultural identity.
